Grade Appeal Request

Use this form to request a grade appeal.

Overview

Students can provide a written statement explaining the basis for the appeal and provide evidence to support it. The form also asks for the following information:

  • Student name
  • Student ID#
  • Course/Section
  • Semester/year
  • Grade received

This form and required evidence shall be submitted to the Executive Vice President of Academic Affairs within 15 days of final grade posting, after meeting with the Faculty member and the Academic Dean of the Department.
